A link to an online intake form will be included in your confirmation email. If this is your first time visiting the clinic, this MUST be filled out prior to your appointment. (If you have been to the clinic before, but your health history needs to be updated, we will send a direct link to your profile in a separate email. Do not fill out a new form)
Cancellation Policy
AL LEAST 24 HOURS notice is required to cancel or reschedule your appointment. Failure to do so will result in a missed appointment fee equal to the cost of the booked appointment. This fee cannot be billed to your insurance and will be due upon receipt via E-transfer or credit card. No Exceptions. By proceeding, you agree to this policy.
Zero Tolerance Policy for Misconduct
Mend Massage Therapy upholds a strict zero tolerance policy for any form of misconduct during massage therapy sessions, including but not limited to:
- Sexual harassment
- Sexual assault
- Indecent exposure or acts
- Verbal or physical abuse of any kind
- Suggestive language or inappropriate comments/requests
If any such behavior occurs:
- The session will end immediately
- The scheduled session will be paid in full
- The client will be permanently discharged from the clinic
- Law enforcement will be contacted where applicable
Massage Therapists are Regulated Health Professionals. Any violation of this policy will be treated with the utmost seriousness.
